Diabetes Management Case Study
This case study explores the management of diabetes in a 45-year-old patient. The patient was diagnosed with Type 2 diabetes and presented with elevated blood glucose levels. A comprehensive management plan was developed, including dietary changes, exercise, and medication.
Patient Background
The patient had a family history of diabetes and was overweight. Initial assessments indicated a need for lifestyle modifications.
Intervention
The intervention included a personalized diet plan focusing on low glycemic index foods, regular physical activity, and the introduction of metformin. The patient was monitored regularly to assess progress.
Conclusion
After six months, the patient achieved significant weight loss and improved blood glucose levels, demonstrating the effectiveness of a multidisciplinary approach to diabetes management.
